Concerned about introducing a character -- (Author) ZetaSonic

Content: One of the characters I’m introducing into a story I’m planning is a kindergarten-aged girl with UI. Problem is that her first scene is in a baby diaper commercial advertising Size 8’s… and she’s the one who gets the change halfway through, and not the one playing her sister.
It’s not played for laughs wholly, but I’m kind of nervous about this subject considering that the in-universe script calls for a bowel movement and a Number One in her shorts due to a miles-long traffic jam.
Furthermore, she wears training pants during the day anyway (the voucher given would allow her parents to buy them for a year at a discount). Is this too insulting or too nasty (I’ve seen just as bad or worse on kids’ shows back in the day), and I’d REALLY hate it if some sicko got off to it when it’s not intended to be titillating (she’s four, people!).
I’m not incontinent whatsoever, but I just need a second opinion about this whole thing.

Content: I still don’t know. It was for an in-universe commercial for baby diapers, and she’s not even the main character (though she is significant) and her disability is far from the main focus of the plot. The thing is that one kid in her class ridicules her over it and another took it upon himself to be her bodyguard.
I talked to a couple of friends about this and they think it sounds like my concerns are overblown and accidents in the backseat just something that’s not unheard of with kids that young.
